Waiver

In order to go on trips with NUHOC, you must fill out this form before each trip:

NUHOC Trip Waiver

Please have the waiver filled out before your trip call (usually takes place on the Thursday night before the weekend of your trip). You will be prompted to log into your Northeastern account in order to fill out the waiver.
